Physical Activity Readiness Questionnaire (PAR-Q)

A PAR-Q is designed to identify the small number of individuals for whom physical activity might be inappropriate, or those individuals who should seek medical advice prior to commencing a programme of physical exercise. 

Following are the questions included in the PAR-Q that we will complete together, prior to commencing your programme of exercise.

 

 

Questions

  1. Have you ever been diagnosed with a heart-related issue, experienced pains in your heart or chest or felt pain in your chest when you do physical exercise?
  2. Have you ever been diagnosed with high blood pressure?
  3. Have you ever been diagnosed with high cholesterol (serum level above 6.2mmol/L)?
  4. Have you ever been diagnosed with Diabetes Mellitus or any other metabolic disease?
  5. Do you ever feel faint or have spells of severe dizziness?
  6. Are you pregnant or have you been pregnant in the last 6 months?
  7. Are you currently taking any drugs or medications?
  8. Have you ever been diagnosed with a bone or joint problem, such as arthritis, that has been aggravated by exercise or might be made worse with exercise?
  9. Have you ever been diagnosed with any back problems or experienced chronic back pain or numbness?
  10. Do you currently smoke?
  11. Are you aware of any other reason why you should not participate in a programme of physical exercise?

 

If you answered NO to all questions above, it gives a general indication that you may participate in a graduated programme of physical exercise.  It does not guarantee that you will have a normal response to exercise.

If you answered YES to any questions above, you must obtain written confirmation from your doctor that you are suitable to commence a programme of physical exercise.


Declaration

I have read, understood and answered the questions above honestly.  Any questions I had were answered to my full satisfaction.

I wish to participate in activities that may include aerobic exercise, resistance exercise and stretching. I realise that my participation in these activities may involve the risk of injury and even the possibility of death.  Furthermore, I confirm that I am voluntarily engaging in an acceptable level of physical exercise and an exercise programme that has been recommended to me.
