Client-Trainer Agreement
Following is the text of the Agreement that we will both sign, prior to commencing your programme of exercise.
Agreement
The following constitutes the entire Agreement between the Client and the Trainer.
The Trainer agrees to use their skills and knowledge to design a programme of exercise that will take into account the Client’s personal goals, fitness level and exercise preferences.
The Trainer will provide coaching, supervision advice and support that the Client may require to help them achieve their goals. The Client’s progress will be regularly monitored and the programme revised and adjusted accordingly.
The Trainer will provide all necessary equipment and will organise appropriate venues for all the training sessions.
It is understood between Client and Trainer that both will commit to the programme and give 100% effort.
It is the Client’s responsibility to wear appropriate clothing and footwear. Clothes should be loose fitting and non-restrictive. Footwear should be comfortable and provide adequate support.
The Client is responsible for bringing an appropriate water bottle, filled ready for the session, and any sustenance required by the Client for pre or post session nutrition.
All Client information will be kept strictly private and confidential. If the Trainer requires further medical information from a practitioner, the Client must provide such details prior to the programme of exercise commencing or continuing.
Terms and Conditions
Medical Screening
The Client must complete a Physical Activity Readiness Questionnaire (PAR-Q) before commencing any programme of exercise with the Trainer and thereafter every 12 months.
Where the Client has answered ‘Yes’ to any PAR-Q questions, the Trainer may require written medical clearance from the Client’s doctor prior to agreeing a programme of exercise. Your medical centre may charge for this service.
The Client must inform the Trainer of any new medical conditions arising between sessions that may not have been covered earlier in the PAR-Q. The Trainer may require that the Client be seen by a doctor prior to recommencing training sessions.
Cancellations
It is the responsibility of the Client to ensure that the dates and times booked are correct. To cancel a booked session, the Client must notify the Trainer either in person or by phone, text or email. Notice of between 24 and 48 hours will require a 50% payment of the session fee. Notice of less than 24 hours notice will incur full payment of the session fee.
In the event of cancellation by the Trainer, the Trainer is responsible for notifying the Client of any such cancellation and there is no charge or loss of session.
Lateness
The Trainer is only required to wait for 20 minutes past the agreed session start time. After this time the Trainer may decide to leave the training location and the Client will forfeit the fee for the session. If the Client arrives within the 20 minutes of the agreed session start time, the Trainer will complete the time left of the original session with no extra time being given.
If the Trainer is more than 10 minutes late on arrival to the session, the Client can have a full hour from the time that the Trainer arrives (if convenient to both Trainer and Client), or the Client can have another session extended by the remaining time.
Fees & Charges
Payment for single sessions must be made at the end of each session.
Block bookings must be paid for in advance however sessions do not have to be booked in advance. Pre-paid block booking sessions must be redeemed within 12 weeks of purchase or they will be forfeited.
Refunds
If the Client is unable to continue a block of sessions for medical reasons, a refund may be available for unused sessions. The Client will submit a letter from their doctor clearly stating exercise restrictions prior to any refund being given. Refunds are not provided under any other circumstances.
Declaration
I, the Client, have read and understand the above Agreement and the Terms and Conditions, and I agree to follow the guidelines set out above. Any questions I had were answered to my full satisfaction.
I acknowledge that I have either had a physical examination and have been given my doctors permission to participate, or that I have decided to participate in the activity and/or use the equipment and machinery without the approval of my doctor and do hereby assume all responsibility for my participation and activities. I release the Trainer from any liability for personal injury or other damage I may sustain whilst engaging in any exercise programme suggested by the Trainer.
